This file has the Waiver of Academic Regulations that has the directions students need to follow and file with CAPP in the event that a Waiver is required. This form is intended to be used only for two purposes: 1) to request a Leave of Absence and 2) to request waiver of a regulation that results in disciplinary action including dismissal from the Doctor of Pharmacy curriculum or probation. This form is not to be used in conjunction with a grade appeal or a waiver of course prerequisites. CAPP does not hear grade appeals or requests for waiver of course prerequisites. These two issues need to be addressed by the instructor of record for each course and appeals from course grades and waiver of prerequisites are to be directed to the Department Chairperson for courses offered by the respective departments.
